We Have Social Workers on Staff and Counseling Services Inside our Facilities
Overall health goes beyond the physical but also focuses on the intersections between medical and mental health, as well as the environmental triggers that could result in chronic health issues. Hometown Health Clinic is proud and fortunate to have a social worker on staff to assist in situations that benefit the overall health and wellness of our community.
We understand that there are often external factors that contribute to physical pain, mental health, financial success, and more. Our social service staff is focused on efforts to provide the best all-around care to as many people in our community as possible.
Social workers may refer patients to counseling services within our facilities, or work with organizations outside of Hometown Health Clinic, to form and achieve resolutions to a variety of problems, including:
- Housing and food insecurity
- Substance abuse treatment
- Uninsured or underinsured status
- Affording medications and financial difficulties
- Chronic health issues and mental healthcare
- Social and spiritual issues
- Other issues needed to enhance the daily lives of our community members
