Hometown Health Clinic is your Patient-Centered Medical Home (PCMH).  This means we team up with you to emphasize team and evidence-based care through communication and coordination.

Our approach is a team approach utilizing providers,  nursing staff,  care coordinators, referrals,  and diet and exercise education to treat the whole person, not just the disease.  Our electronic health record allows us to use real time clinical decision support and best practices from the USPST for clinical support on medication and preventive measures.

As a patient you’ll be kept up to date with all pertinent information about your conditions, progress, and screening results. Sign up for our patient portal for quick and easy access to your test results, make appointments,  request medication refills or to message your provider.  To help, be an active team player.  Tell your team about other healthcare professionals who care for you, set goals with your provider that you can reach, and make sure you know how to follow the plan that you and your provider work out.  Let your team know what is working and what is not working so that we can make changes together.
